This questionnaire will take about 15 to 20 minutes of your time. Here are some things you may be asked:
- specific dates of birth, of immigration, or of marriage for both yourself and your parents
- your parents’ marital status at the time you were born
- information on whether you or your parents have ever held citizenship from other countries
- whether your parents were Canadian when you were born
- whether you or your parents were ever issued citizenship documents.

In certain complex cases, this questionnaire may not be able to determine whether a person is a Canadian citizen. It is important to note that people who were Canadian citizens the day before the new law came into effect keep their citizenship.
