  • specific dates of birth, of immigration, or of marriage for both yourself and your parents
  • your parents’ marital status at the time you were born
  • information on whether you or your parents have ever held citizenship from other countries
  • whether your parents were Canadian when you were born
  • whether you or your parents were ever issued citizenship documents.

In certain complex cases, this questionnaire may not be able to determine whether a person is a Canadian citizen. It is important to note that people who were Canadian citizens the day before the new law came into effect keep their citizenship.

Should you decide to make an application for proof of citizenship and pay the prescribed fee, your application will be assessed by a citizenship officer, in accordance with the requirements of the Citizenship Act, without regard to any outcome you may have attained through completion of this informal questionnaire.
