This form is for you if you are a permanent resident who:
- Provided your Canadian mailing address to CIC at the point of entry
into Canada and have not received your Permanent Resident Card after
at least four weeks.
or - Provided your Canadian mailing address to CIC sometime after entering
Canada and have not received your Permanent Resident Card after at least
four weeks. If you are a permanent resident, you have a maximum of 180 days
to provide your Canadian address after entering the country.
or - Possessed a valid Permanent Resident Card that was lost, stolen or destroyed but you do not wish to reapply for a new card immediately. You can obtain a PR Card application kit from this site.
