Payment can be made by cheque or money order made payable to the Receiver General for Canada.

Note: Your request will not be processed until your application fee is received.

• Download your request form and carefully read the instructions
• Complete, sign and date your request form
• Fill out the Consent for an Access to Information and Personal Information Request form.
• Pay the fees (if applicable)
• Mail your fully completed request forms with the supporting documents to the following address:
