How to make a request under the Privacy Act

A request for personal information held by Citizenship and Immigration Canada (CIC) should be made to CIC under the Privacy Act.

What you will need to submit a request

  1. Formal request:

  2. Proof that you have the right to make a request:
    • To be eligible, you must be a Canadian citizen, a permanent resident of Canada or an individual currently present in Canada.
  3. Consent(s), if applicable:

    Note: Please note that we will no longer be accepting the Authority to Release Personal Information to a Designated IndividualIMM 5475 or the Use of RepresentativeIMM 5476 for ATIP purposes.

Note: There are no fees involved with the processing of a request under the Privacy Act.

Where to send requests

Requests made pursuant to the Access to Information Act can be sent electronically if using the ATIP Online Request tool.

To submit a written request:

  • Download your request form and carefully read the instructions
  • Complete, sign and date your request form
  • Mail your fully completed request form with the supporting documents to the following address:
