All DMPs should be aware of and follow the administrative procedures relating to the immigration medical examination (IME) outlined below.
DMPs must provide appointments to applicants requesting an IME within 14 days of the request.
At the time of booking, DMPs should also remind applicants what to bring to the medical exam appointment.
DMPs must forward all immigration medical reports to the regional medical office in their area by mail. Do not send immigration medical reports to the visa office processing the applicant’s file. This will only increase the processing time.
Medical records must be sent within five days of the receipt of all required information. In urgent situations, DMPs may request that medical records be expedited. DMPs may forward these records by commercial courier, at the applicant’s expense.
Applicants are responsible for all shipping costs.
With the exception of refugees and refugee claimants, applicants must pay all fees and costs associated with the immigration medical exam, including costs for DMP services, and lab and radiology examinations.
DMPs should charge reasonable fees based on discussions with the regional medical officer for their area. The fee schedule for the immigration medical exam, and lab and radiology services should be posted in the clinic, and copies should be made available to applicants who request them.

The numeric designation of the forms is located in the bottom left-hand corner of the first page.
Different IMM 1017 forms are used depending on applicants’ immigration classification and whether or not they are excessive demand exempt.
Applicants should arrive at their immigration medical exam appointment with the IMM 1017, except those requiring an up-front medical exam. The IMM 1017 is usually sent to the applicant by the visa office.

Up-front medical exams are performed when applicants arrive at their immigration medical exam appointment with a completed Appendix C – Medical Instructions form.
The Medical Report: Section A (IMM 1017 EFC) is used for up-front medical exams. DMPs should have a supply of IMM 1017 EFC forms on hand. To order forms, see the instructions in the table above.
Once completed, all immigration medical forms and reports are the property of Citizenship and Immigration Canada. DMPs must send immigration medical exams to their regional medical office.
DMPs must never give a copy of the immigration medical examination documents to applicants or their representatives. The only document that is returned to the applicant is the second copy of the Medical Report: Section A (IMM 1017 EFC).
If applicants want a copy of the medical exam documents, they must submit an official request to Citizenship and Immigration Canada’s Access to Information and Privacy division.
DMPs may be required to maintain their own copies of the immigration medical exam forms and records in accordance with local regulatory and licensing requirements.
