This application is for permanent residents or Canadian citizens who wish to correct errors made by Canadian immigration officials when recording certain personal information on the Record of Landing, Confirmation of Permanent Residence or temporary resident documents.

Historical documents, such as the Record of Landing and Confirmation of Permanent Residence, will not be amended to reflect changes in your civil status situation since they were issued to you, for example, a name change due to marriage.

Send this application to the CIC Query Response Centre in Ottawa. You can find the address in the Document Checklist  that comes with the application.

• Complete, sign and date your application forms.
• Mail your fully completed application.
