This application includes the following items:

1. Application for a Citizenship Certificate (including the Document Checklist) [Form CIT 0001]  (PDF, 319 KB)
2. Citizenship Photo Specifications
3. Fees Receipt
   You have two options to obtain a fees receipt:
   1. Pay your fees online.
   2. Pay your fees at a financial institution in Canada.
      You will need an original fee receipt
4. Instruction Guide [CIT 0001]

This application is for Canadian citizens residing inside or outside Canada who wish to apply for a citizenship certificate as proof of Canadian citizenship. Supporting documents required will depend on whether the application is for a new certificate or to replace one that was lost, stolen, damaged or destroyed.

In Canada.

• Read your instruction guide and download your application forms
• Complete, sign and date your application forms
• Pay the fees
• Mail your fully completed application to the Case Processing Centre in Sydney, Nova Scotia

Outside Canada. Contact the nearest Canadian embassy, high commission or consulate.
