This application is for persons who are eligible for the Interim Federal Health Program (IFHP) and who want to apply for coverage under this program.

The accompanying guide contains important information on eligibility requirements and the procedure to follow when applying for the IFHP coverage. A list of Citizenship and Immigration centres that process this type of application in Canada is included. Refer to it for the address of the CIC to which you must send your application.

• Read your instruction guide and download your application forms.
• Complete, sign and date your application forms.
• Pay the fees.
• Mail your fully completed application.
