This form is for people who wish to authorize Citizenship and Immigration Canada to release their personal information to an individual they designate.

The individual you designate will be able to obtain information on your case file, such as the status of your application. However, he or she will not be a representative who can conduct business on your behalf with CIC.

A person must be appointed as a representative to conduct business on your behalf with CIC.

If you have not yet submitted your application, send this form along with your application to the office listed in your respective application kit.

If you have already submitted your application, send this form to the office where you submitted your original application.

• Read your instruction guide and download your application forms.
• Complete, sign and date your application forms.
• Pay the fees.
• Mail your fully completed application.
