How and where do I obtain one?

If You Live in the Slovak Republic
You may obtain a Výpis z Registra Trestov in person or through an authorized representative from any office of the public prosecution or any municipality with register by submitting:

• an application form;
• an original identification card; and
• a stamp-duty mark covering the processing fees.

Foreign nationals must also present their original passport and a certified translation of their birth certificate.
